Southcentral LTC-RISE Program

Long-Term Care (LTC) facilities have experienced severe devastation from the COVID-19 pandemic. The challenges of caring for our most vulnerable populations in this setting, while managing the threat of COVID-19, creates extremely challenging and stressful circumstances.

It is crucial that LTC facilities receive continued support as they both battle the COVID-19 pandemic and work to recover and rebuild.

The Southcentral LTC-RISE team at Penn State Health is responsible for coverage of 13 PA counties in this region including: Adams, Bedford, Blair, Cumberland, Dauphin, Franklin, Fulton, Huntingdon, Juniata, Lebanon, Mifflin, Perry, and York.

Our Goal
is to provide LTC facilities with access to key resources that include:
  • COVID-19 support
  • workforce development
  • staff education
  • quality improvement
  • emergency management
  • coaching
  • wellness and community
  • resource building
